Providing families and loved ones an inclusive care experience through resources and ongoing supports for the aging populations.

In-Home Caregiving Services

We take the time to meet with each client and develop a flexible and customized Care Plan that allows for the highest level of independence, while still ensuring your loved one’s safety. We can assist with several types of living situations such as in-home, independent living, assisted living, living with family or friends, as well as post-rehab stays. PNW Aging hires certified, skilled and compassionate Caregivers.

Case Management

Our goal as Case Managers is to fill the role of the kind and knowledgeable family member or friend, who would if they were available. We can help you work through the many different challenges you face. PNW Aging Case Managers establish a “one-on-one” relationship with you and your loved ones. It is our responsibility to listen and get to know you, so we can understand what kind of assistance is needed and what you would like to happen. From there we act as your advocate and your healthcare liaison.

Healthcare Power of Attorney

Many have heard the term “adult orphan” and wonder what this means. More and more people are realizing they need someone to be there for them when they realize they do not have children or family members who can step in and legally advocate for them when they are no longer able to advocate for themselves. PNW Aging acts as a legal healthcare decision maker, ensuring their wishes are followed and their care needs are addressed as they indicated in their estate planning documents. As healthcare professionals, we can put your mind at ease knowing you have support and an advocate to ensure your wishes are followed through with.
